Diabetes Team - Nathaniel Tan:

Nathaniel is a Registered Nurse (RN) who has completed a 4-year bachelor’s degree in Nursing from York University in June 2022. He has been working with the Diabetes Team since April 2023 and is currently in the process of becoming a certified diabetes educator (CDE). He has an Intensive Insulin Therapy certificate from Michener Institute University and now specializes in diabetes prevention and management for our patients

Diabetes Team - Samantha Matte:

Sam is a Registered Dietitian (RD) and a Certified Diabetes Educator (CDE) who has completed a 4-year Honours Specialization program in Nutrition and Dietetics at Western University in 2020. She has also completed her 1-year Dietetic Education and Practical Training program in 2021 at Western University. Prior to joining our team in 2023, Sam has worked in both Long-Term Care and Primary Care as a Registered Dietitian. Sam’s current area of focus is in diabetes care, and has become CDE certified in 2024. Sam is very passionate in helping patients take control of their health and well-being, and does so using evidenced-based nutrition therapy.
